Patients MUST be able to use email, complete digital forms, and navigate video visits independently ...
OR
They MUST have another responsible adult to assist them with these things (spouse, adult child, caregiver, POA, etc.)
Evaluations Include:
- Online cognitive testing (you'll be sent a link that allows you to complete the testing in the comfort of your home and at a time that works best for you)
- A 1-hour telemed visit during which the provider will review any records you have provided, review the test results with you, and explain their conclusions and recommendations
- A letter or report summarizing the results and recommendations for your records, and for you to share with your other providers if you so desire (typically available within a week following the visit)
Insurance and Fees
Evaluations are a flat fee of $400
- Contact your insurance company at the number on the back of your card to find out if Clear Mind Coaching LLC and Dr. Matthew Bridgman are in their network. If "yes", we can submit bills directly to your insurance.
- If we are out-of-network with your plan, you will be responsible for the cost of the evaluation, due at the time of the evaluation. We may help you obtain reimbursement from your insurance for a portion of the cost if your insurance plan includes out-of-network benefits.
To Request An Evaluation ...
You, a family member, or your healthcare provider may request an evaluation using the button below. You will be asked to provide some initial information. Upon submitting your request, you will receive an email in 1-2 business days guiding you to set up a TherapyPortal, through which you will complete a variety of intake forms. Once those are complete, you will receive a message through that portal with instructions for accessing the online test and scheduling the 1 hour telemed visit with the provider.
In What State Do You Live?
Please note, Dr. Bridgman is licensed to provide services in Pennsylvania directly, and has authority to practice telepsychology in most other states through the PsyPACT Commission (see https://psypact.gov/page/psypactmap). However, at this time the following states are not part of PsyPACT and therefore Dr. Bridgman is not able to provide services to residents of these states: Alaska, California, Hawaii, Iowa, Louisiana, Massachusetts, New Mexico, New York, Guam, Oregon, Puerto Rico, and the U.S. Virgin Islands.
